10 CERTIFICATE The APC Certificate is issued to candidates who have successfully completed the Stream A or Stream B process, and is accepted for application for registration by all state and territory pharmacy registering authorities in Australia. A certificate will be issued to the successful candidate through the appropriate pharmacy registering authority indicating that the candidate has satisfied the APC's requirements and completed its examination procedure. It is not a registration certificate. The candidate will be required to achieve a pass in the Stage I Examination and satisfactorily complete: a supervised practice period; a pre-registration training program; a pass in the NFEC Examination and in all three components of the Stage II Examination; and possess a current first aid certificate. Candidates should note that all registering authorities reserve the right to specify additional requirements including additional supervised practice and or examinations ; which an applicant must satisfy in order to obtain registration. O BJECTIVES . Alternative joint surfaces have been developed to reduce the problem of PE wear causing periprosthetic osteolysis and aseptic loosening following Total Hip Arthroplasty THA ; . This study examined differences in the amount of PE wear with the use of metal or ceramic heads. M ETHODS . The study comprised 92 hips with uncemented THA of the same prosthesis type BICONTACT-Aesculap, Tutlingen ; with a minimum 5-year follow-up. Conventional PE was used in all cases. All femoral heads were 28 mm in diameter and ceramic Group I ; in 69 hips 75% ; and cobalt-chrome alloy Group II ; in 23 hips 25% ; . The two groups were of similar age, sex ratio, activity level, body mass index and follow-up period. The location of the acetabular component was determined according to the Ranawat method. The Dorr method was used to measure linear wear in the polyethylene. R ESULTS . The Harris hip score rose from average 52 35-65 ; to 89 76-98 ; in Group I and in Group II from 45 26-59 ; to 87 79-97 ; p 0.05 ; . The annual PE wear was an average of 0.16 mm 0.006-0.44 ; in Group I and in Group II 0.25 mm 0.08-0.48 ; the difference being statistically significant p 0.001 ; . In Group I, the location of the acetabular component was in place in 53 hips 76.8% ; , above 5-9 mm ; in 11 hips 15.9% ; , and lower 5-8 mm ; in 5 hips 7.3% ; . In Group II, these values were 17 73.9% ; , 4 17.3% ; 5-8 mm ; , and 2 8.8% ; 5-9 mm ; respectively. The acetabular index was average 43 40-48 ; in Group I and 41 39-47 ; in Group II p 0.05 ; . C ONCLUSIONS . We conclude that although there is no difference between the groups with THA from the clinical results aspect, the annual amount of PE wear is less in cases where a ceramic femoral head is used and therefore this should be preferred. As a result, the tailing pond located in a lead ore refinery plant was flooded and lead-contaminated water spilled into Clitty Creek; exposing the village downstream to large amounts of lead. Subsequently, soil deposits around the plant was found to contain 5, 800-102, 574 mg kg of lead, and marine life in Lower Clitty Creek contained between 5.45 to 81.79 mg kg dry weight of lead. The Ministry of Public Health, together with the Ministry of Science, the Ministry of Industry, and officials from the Kanchanaburi Office of Public Health, began measures for environmental deleading and assessment of exposure and health risks of the population. Methods: This is a retrospective cohort study of the effects that environmental remediation alone had on the blood lead levels of children residing in Lower Clitty Creek Village during the period between 1997-2000. Sixty-eight children were followed yearly for their blood lead levels and hematocrit, beginning in 1997. Simultaneously, programs for environmental remediation had begun. Methods for environmental decontamination included: shutting down the mine and refinery plant that was the source of lead, preventing contaminated soils from entering into populated areas, creating a lead-free water source, and providing alternative source of protein and avoiding consumption of animal meat from the creek. Results: The average age of children who participated in the follow up program was 3.39 + 1.69 years. The average BLL prior to initiation of environmental remediation was 27.75 + 5.4 mg dl. After environmental remediation began, BLL at one year 1998 ; was 30.64 + 4.49 mg dl p 0.072 ; , and at two years 1999 ; was 30.30 + 5.1 mg dl p 0.537 ; . Neither value showed statistically significant differences from the value before deleading measures had taken place. Discussion: Environmental decontamination is considered to play a key role in the treatment of chronic low dose lead exposure. It is recommended as first line treatment in cases of lead poisoning from the environment. However, in our case, such measure did not lead to the expected decrease in blood lead levels of children in the area. Thus, chelation therapy should be considered in this case, in order to prevent the long term detrimental effects of lead poisoning.
