The purpose of this study was to understand how key stakeholders view the concept of "ease of extractability" and explore the feasibility of developing a scale that could reliably differentiate the "ease of extractability" of active product from various modified-release opioids MROs ; using a concept mapping methodology. METHODS: A 3-stage process utilizing the structured qualitative research method of concept mapping is applied to the development of a scale that could reliably differentiate the "ease of extractability" of active product from various MROs. In the first stage Pre-Concept Mapping ; , qualitative interviews were conducted with key stakeholders including opioid abusers with experience extracting active product from MROs, opioid abuse specialists, former DEA personnel, and substance abuse counselors ; in order to 1 ; define the components of "extractability" and 2 ; define boundaries of the "ease of extractability" rating scale. In the second stage Concept Mapping ; , a more formal process of obtaining ratings and concept groupings from stakeholders was conducted. Concept mapping data were analyzed using multidimensional scaling and hierarchical cluster analysis. These results were used to develop a preliminary version of the scale. In the third stage Validation and Reliability ; , the scale will be empirically tested for reliability and validity, resulting in the final version of the scale. RESULTS: Stakeholders agreed on specific dimensions to include as part of the ease of extractability rating scale. Stakeholders defined extraction of the active ingredient from MROs as the process of manipulating biting, crushing, cutting open, cooking, etc. ; a product to extract the active ingredient or prepare it for administration. CONCLUSION: Concept mapping was useful in operationally defining the concept of "ease of extractability" and for exploring the feasibility and development of a scale to reliably differentiate the "ease of extractability" of active product from various MROs. Polysaccharide " Our results demonstrate that the inhibitory compound is released upon exposure of A. anophagefirens to amylase. That this inhibitory compound is similar to that released upon exposure of intact A. anophag&vens to bivalve gills is indicated by the blockage of inhibition by pre-treatment of the gills with ergometrine in both instances. The time for inhibition of clearance rates in bivalves exposed to A. anophugefirens is much faster than the time for inhibition of the lateral cilia of isolated gills. Tracey 1988 ; found inhibition of clearance rates after only 20 min of exposure to A. anophagefirens, whereas we found that the time for inhibition was much slower in isolated gills. These differences should, in fact, be expected for the following reason. The ingestion of A. anophagefferens by whole animals would result in the faster digestion of the extracellular coat in comparison to the digestion by isolated gills because digestion would occur not only on the gills but also in the digestive system of a whole animal. In summary, we propose that the inhibition of clearance in bivalve mollusks exposed to A. anophugefirens is brought about by the digestion of the extracellular coat, releasing a dopamine-mimetic compound, which brings about the arrest of the lateral cilia in certain species.
