In this update, beta blockade is highly recommended in asymptomatic patients with structural damage. Furthermore, in patients with symptomatic heart failure, beta blockers have been shown to reduce morbidity and mortality. This is significant since heart failure continues to increase in incidence and prevalence and is the number one reason for hospital admissions in the Medicare population. PURPOSE: To determine whether heart failure patients in the VA Chicago Health Care System have active prescriptions for beta blockers and if doses are being optimized according to the ACC AHA guidelines. METHODS: Four hundred thirty five patients with a documented inpatient or outpatient diagnosis of heart failure from September 1, 2001 to September 30, 2002 were randomly selected for this retrospective analysis. Only patients with left ventricular systolic dysfunction and ejection fraction less than or equal to 40% were eligible. For approximately two hundred patients meeting this entry criteria, the following information was recorded from patients' electronic charts: 1 ; demographic data, 2 ; beta blocker prescribed; initial and current dosage regimen, 3 ; ACE inhibitor usage current dose, 4 ; other heart failure medications, 5 ; heart failure hospitalizations, 6 ; other hospitalizations, and 7 ; death. The test for protein in the aqueous humor, employed only in the present study, is particularly important. The bloodaqueous barrier is very easily broken down and then protein seeps into the aqueous.
