The diagnosis of leptospirosis were based on the results of the serologic tests as well as on the clinical data obtained. Increasing titres of the first and second serum samples were considered diagnostic. Initially high titres of 1: 000 or more were also considered indicative of infection when supported by clinical data. Two cases did not have any serologic tests. However, since they presented typical clinical pictures and no other etiologic cause could be demonstrated by laboratory tests, we considered them to be cases of leptospirosis and included them in our study. RESULTS AND DISCUSSION Age and Sex The distribution of cases according to age and sex group is shown in Table 1. Our youngest patient was 18 years old, while the oldest was 50 years. Walch-Sorgdrager reported that the majority of their cases were between the ages of ten and forty, 4 and Sulit reported 38% within the same age group. We had essentially the same proportion. Our patients were predominantly male and this again agrees with other reports in literature.4, 5 This male preponderance is believed to be due to occupation rather than to be a sex-linked susceptibility and arixtra.

Treatment trends Drugs for the treatment of cancer are the largest area for new drug development, with hundreds of new cancer drugs and new indications for existing cancer drugs in clinical development. From a treatment standpoint, cancer is becoming a more chronic condition. Many of the newer and more targeted drugs are better tolerated, and they are often administered for months or years until there is disease progression. Unlike drugs of the past, which were so toxic that they could only be used for a few cycles, many of the newer drugs are used as a form of maintenance treatment. Treatment costs will continue to expand as these drugs are used on a longer-term basis, and in combination regimens that can cost , 000 to , 000 for a month of treatment. Many new cancer drugs are incremental to current treatments, so they may be significant drivers of new utilization. Careful monitoring and management of cancer treatment costs will be essential. Specialty pharmacy distribution of these drugs can provide an effective solution for managing costs by limiting experimental uses and driving larger and more consistent discounts. New oral drugs Orally administered cancer drugs will be increasingly important trend drivers over the next few years, given the large number of new drugs in the pipeline and the likelihood of expanded indications and off-label usage for existing drugs. Many of these drugs are likely to be more expensive than current treatments, and many will be used in addition to current treatments. Recently approved and potential products include: Nexavar sorafenib ; was approved in December 2005 for the treatment of advanced renal cancer, and Sutent sunitinib ; was approved in January 2006 for advanced renal cancer and gastrointestinal stromal tumors. Each drug costs in the range of , 000 to , 000 per month. Revlimid lenalidomide ; received approval in December 2005 for reducing transfusion requirements in patients with myelodysplastic syndrome. The primary impact of this drug on trend is likely to be in the treatment of multiple myeloma; the FDA has recently granted a priority review for this indication. Orally administered cancer drugs in development include dasatinib, vatalanib, atrasentan, and satraplatin. Dasatinib, an oral tyrosine kinase inhibitor, is likely to be approved in 2006 for the treatment of chronic myelogenous leukemia that is refractory to Gleevec. Supportive treatments Supportive care therapies--such as Epogen epoetin alfa ; , Aranesp darbepoetin alfa ; , Neupogen filgrastim ; , and Neulasta pegfilgrastim ; --represent a significant share of the medication costs for cancer treatment, and they are a significant driver of trend. Utilization of these agents to treat chemotherapy-induced anemia and neutropenia is expected to grow rapidly over the next 3 years. A continuing shift from Neupogen to Neulasta is likely to increase unit costs for chemotherapy-induced neutropenia treatment. The NDA for continuous erythropoietin receptor activator CERA ; was submitted in December 2005. This presentation will impact the forensic community and or humanity by providing a relatively quick and easy method for evaluation of firearm injuries in skeletal remains. The goal of this presentation is to introduce forensic scientists to a technique that can aid in the determination of range of fire in skeletal remains found with firearm injuries. Introduction: The scenario is not an uncommon one: a person goes off into a secluded area with the purpose of commiting suicide by shooting themself, and is later found in a decomposed or skeletal state. The presence of a weapon would certainly indicate a suicide; however, circumstances are not always as they appear. Remains can also be scattered and separated from the weapon, and weapons can be separated from the remains due to theft. When remains are decomposed or skeletal, the usual clues to range of fire, soot, and stippling are often lost or obscured. The authors present a technique that can aid in the determination of range of fire in the absence of visible soot. The sodium rhodizonate staining technique is widely employed in crime laboratories to detect lead residue on clothing. The staining pattern obtained can then be compared to the implicated weapon and ammunition utilized to determine the possible range of fire. This technique employs spraying the garment with sodium rhodizonate, then sequentially overspraying with buffer and then hydrochloric acid. A pink color with buffer and then blue-purple color with acid is indicative of the presence of lead. This test has been employed on skin; however the authors demonstrate the technique on human and animal skulls. Materials and Methods: Amputated heads from six previously slaughtered pigs were purchased for this experiment. Three different weapons and ammunition were utilized: a revolver with a non-jacketed bullet, a 9mm semi-automatic pistol with a copper-jacketed bullet, and a shotgun with 00 buckshot. Hard contact shots were fired with the weapon placed between the eyes on the upper portion of the snout. Distant shots were fired from 3 feet with the handguns and approximately 28 feet with the shotgun. The skin from the head was then removed and tested for traces of lead. The skulls were then boiled in water to aid in the removal of the remainder of the soft tissue. Once the soft tissues were removed, the skulls were re-assembled, if necessary, and tested for lead residue. All skulls were examined for visible lead prior to testing with sodium rhodizonate. Color changes were documented and photographed. A test was determined to be positive for lead if the color changed from pink with the buffer to purpleviolet with the acid. Results: The distant gunshot wound from the revolver with the unjacketed bullet showed faint positive staining of the bone around the outer surface of the defect, with no staining on the inside of the skull. Evaluation of the skin revealed a inch x inch area of positive staining with a few positive spots 6 inches from the wound. The distant gunshot wound from the semi-automatic weapon revealed no visible residue prior to testing. After testing, a small rim of lead partially encircled the skin wound and there were a few positive spots 4, 5, and 6 inches from the wound edges. There were no traces of lead on the skull. The shotgun inflicted multiple buck shot wounds which tested positive for traces of lead on the skin, but not the skull. All contact range wounds had visible residue on the skin prior to testing, and subsequently had brightly positive staining after testing with sodium rhodizonate. The outer surfaces of the skulls also showed positive staining, however the inner surfaces of the skulls also stained positively. Of note, the brains of the pigs were present at the time of the shooting. This finding was reproduced on two human skulls, one of known contact range and the other of presumed contact range. Conclusion: Sodium rhodizonate may help determine range of fire in skeletal remains. Lead residue was detected inside the skulls with inflicted contact range firearm injuries, whereas it was only on the surface of the entrance wound, and in small amounts, of the distant wounds.
